Request a Box of Kindness
You can request a ChemoHero Box of Kindness if you are about to start cancer treatment, or if you are requesting a box for a loved one who is starting treatment.
There are two request forms.
One is for people requesting a box for themselves.
The other is for people requesting a box for a loved one.
If you are receiving treatment in the Seamoor Unit at North Devon District Hospital, you will automatically receive a box at the start of your treatment.
If you are not being treated at the Seamoor Unit, please read the criteria below before completing a form to make sure your request meets our guidelines.
Requests must be for:
• People receiving systemic cancer treatment such as chemotherapy, immunotherapy, targeted therapy, or hormonal therapy
• People living in North Devon who are having treatment outside the area
• Patients receiving chemotherapy at home in the North Devon area
• Patients outside North Devon. We can only send a limited number of boxes across the UK each month
• UK residents only. We are unable to send boxes overseas due to postage costs
Our Boxes of Kindness are designed for people at the start of their treatment. They are not intended for people who have already completed treatment.
Postage information
Each box weighs just over 4kg and costs £7.05 to post using Royal Mail Tracked 48.
At the end of the form you will be asked if you are willing to cover the cost of postage.
Each Box of Kindness contains around £200 worth of useful and comforting items. While the postage cost may seem high, the box is packed to provide meaningful and practical support at a difficult time.
Thank you for taking the time to read this before making your request below.
